How to Register
Joining Coventry GP is quick and easy.
Step 1
Complete our online registration form.
Step 2
Complete the New Patient Health Questionnaire.
Step 3
Our reception team will process your registration.
Step 4
If appropriate, you may be invited to attend a New Patient Health Check with one of our Healthcare Assistants or Nurses.

Do I Need Identification?
You do not need the following to register with a GP practice in England:
An NHS number
Photo identification
Proof of address
Proof of immigration status
If you have these documents available, providing them may help us process your registration more quickly, but they are not required.
New Patient Health Check
Some patients may be invited to attend a New Patient Health Check after registering.
This appointment allows us to:
- Review your medical history
- Update your medication records
- Identify any ongoing healthcare needs
- Arrange any necessary monitoring
- Ensure your medical records are accurate
These appointments are particularly beneficial for patients with long-term health conditions or those taking regular medication.
Online Services
Once your registration has been completed, you’ll be able to access a range of convenient online services, including:
- Rapid Health online requests
- Repeat prescription requests
- Appointment requests
- Test results
- Medical record access
- Administrative requests
We also encourage all patients to download the NHS App, which provides secure access to many NHS services from your smartphone or tablet.
Temporary Registration
If you are staying in Coventry temporarily and require medical treatment, you may be able to register as a temporary patient.

Children can be registered with the practice by a parent or legal guardian.
If available, it is helpful to have:
- Your child’s NHS number
- Details of their previous GP practice
- Child Health Record (Red Book)
If you do not have this information, our team will still be happy to assist you with the registration process.

How long does registration take?
Most registrations are processed within a few working days. Your medical records will then be transferred securely from your previous GP practice where applicable.
Will my medical records transfer automatically?
Yes. Once your registration has been completed, your NHS medical records will usually be transferred electronically from your previous GP practice.
Can I register if I have recently moved to the UK?
Yes. Everyone living in England is entitled to register with a GP. You do not need proof of address, immigration status or an NHS number to request registration.
What if I need medical help before my registration is complete?
If you require urgent medical advice before your registration has been processed, please contact NHS 111 or seek urgent medical care where appropriate.
