At Coventry GP, we use Rapid Health to help patients contact the practice quickly, safely and conveniently online.

Rapid Health allows patients to submit medical or administrative requests without needing to wait on the telephone, helping patients access the most appropriate care more efficiently.

Simply just ‘CLICK’ on the surgery you are registered at.

Booking an appointment is quick and easy. Simply follow the steps below:

Click on Jubilee Health Centre or Broad Street Surgery, depending on which practice you are registered with. This will take you to our Rapid Health online triage system.

New medical problem or symptoms

Select Adult Medical Request to tell us about a new health concern or symptom. Your request will be clinically assessed, and you will be offered the most appropriate care.

Questions, follow-up or administrative requests

Select Questions and Admin if you need to:

  • Request a medical letter or report
  • Request a fit note (sick note)
  • Order a repeat prescription
  • Complete a travel risk assessment
  • Update your personal details
  • Ask a non-urgent question about your care
  • Follow up on an existing problem or test result

Please enter your full name, date of birth, postcode, telephone number, and email address. This helps us identify your records accurately and contact you if required.

Please answer all questions as fully and accurately as possible. The more information you provide, the better we can assess your request and ensure you receive the most appropriate care.

To help us respond as quickly as possible, please do not submit multiple requests for the same problem, as this may delay your care.

Important – Some Appointments Must Still Be Booked by Telephone

Some services cannot be booked through Rapid Health and must be arranged by contacting the practice directly. These include:

  • Cervical screening (smears)
  • Childhood and travel vaccinations
  • Blood tests
  • NHS Health Checks
  • Wound care and dressings
  • Long-term condition reviews (where advised)
  • Appointments requested directly by a clinician
  • Any appointment specifically advised by the practice to be booked by telephone

Please avoid sending multiple requests for the same issue, as this may delay your care.


If you or someone else has a life-threatening medical emergency, such as chest pain, severe difficulty breathing, signs of a stroke, heavy bleeding, or loss of consciousness, call 999 immediately or attend your nearest Emergency Department.

If you need urgent medical advice when the practice is closed, or you are unsure where to go, please contact NHS 111. They are available 24 hours a day and can advise you on the most appropriate care.

For all routine and non-urgent medical requests, please use our Rapid Health online triage system. Every request is clinically assessed, and you will be directed to the most appropriate member of our team or offered an appointment where clinically indicated.
